Winter Park, FL – Injured workers who are discharged from a hospital and need skilled care that cannot be delivered in the home are admitted into a post-acute care, rehabilitation or skilled nursing center. PACC Coordinators identify the most appropriate facility, provide a treatment plan and monitor ongoing treatment, sending weekly updates to claims representatives.
